Fees
Tomoka Surgery Center charges a facility fee for each surgical procedure performed by your surgeon.
The Facility fee includes:
Use of procedure, operating and recovery rooms
Nursing care
Medications and standard supplies
The Facility fee DOES NOT include:
Physician Fees
Anesthesiologist Fees (if one is needed for your procedure).
Other professional services you receive while you are at the surgery center, such as laboratory and pathology fees.
You will receive separate bills for these services.
Payments
We require payment of your deductible, co-payment and co-insurance at the time of admission.
Cash, Checks, Money Orders, Debit, MasterCard, Visa, American Express and Discover are accepted forms of payment.
Medical Insurance
The Center accepts most major medical commercial insurance, HMO/PPO plans, Medicare, Medicaid, and Worker’s Compensation.
We will bill your insurance company as a courtesy; however, any balance due is your responsibility. Your Medicare and/or insurance cards are required at time of admission.
Tomoka Surgery Center will make every effort to contact your insurance company prior to your procedure to verify your coverage. This verification does not constitute a guarantee of payment by your insurance company.
All co-pays, deductibles and co-insurance are due at the time of admission as determined by your insurance benefits pertaining to ambulatory surgery centers.
Self pay patients will be required to pay for services on or before the admission date unless other arrangements have been made with the Center. If special payment arrangements are necessary, please call the Business Office prior to the day of surgery at (386) 615-7584.

Payment Plans
Each patient is expected to pay the estimated financial liability on or before the day of service. If special payment arrangements are necessary, please call the Business Office prior to the day of surgery. We accept CareCredit healthcare credit card with special financing options.
Collection Procedures
Patients are expected to pay their financial obligations in a timely manner including the estimated portion by the day services are received, and any remaining portion upon finalization of the claim by the payer. Unpaid claims by the payer may result in the account’s outstanding balance being fully transferred to the patient for collection. If the account becomes delinquent, it may be placed with an agency for collection in which their fees and expenses may be the obligation of the patient.

Services may be provided in this health care facility by the facility as well as by other health care providers who may separately bill the patient and who may or may not participate with the same health insurers or health maintenance organizations as the facility.
Patients and prospective patients may request from this facility and other health care providers a more personalized estimate of charges and other information. Patients and prospective patients should contact each health care practitioner who will provide services in this surgery center to determine the health insurers and health maintenance organizations with which the health care practitioner participates as a network provider or preferred provider.

In compliance with Florida Law, FS 395.301(6),
Tomoka Surgery Center has established an internal process for reviewing and responding to grievances from patients. Patients disputing a charge that appear on the patient’s statement must formally file a grievance in writing. To initiate the process for submitting a grievance, the patient should contact TSC at 386-615-7584. The patient must provide their name and contact information and the Administrator will respond within 7 business days to start the process of reaching a resolution.
